Perimenopause Panel

Irregular cycles, poor sleep, hot flashes, mood changes and low energy are common in the years before menopause — and several of them overlap with thyroid changes. This panel checks the hormones behind the transition and your thyroid in one draw.

What's included

  • FSH & LH

The signals your brain sends to the ovaries. FSH tends to rise during the transition.

  • Estradiol, ultrasensitive (LC/MS)

Your main estrogen, measured with a method that stays accurate at the low levels common in perimenopause.

  • Progesterone (LC/MS)

Often one of the first hormones to change.

  • TSH with reflex to Free T4

Thyroid function — thyroid changes can feel almost identical to perimenopause.

  • Ferritin

Iron stores, which heavy or irregular periods can lower.

  • Vitamin D

Commonly low, and tracked alongside bone health.

  • Lipid panel

Cholesterol often shifts during the transition.

  • Hemoglobin A1c

Average blood sugar over about three months.

  • CBC and Comprehensive Metabolic Panel

Blood counts, kidney and liver function, electrolytes.

How to prepare

  • Fast for 8–12 hours before your draw. Water is fine.
  • If you still have periods, get tested on cycle day 2–5 (day 1 is the first day of full flow, not spotting).
  • If your cycles are irregular or have stopped, test whenever it's convenient.

Why one test isn't the final word

Hormones during perimenopause can swing week to week, so a single draw is a snapshot. Testing again every few months shows the direction things are moving, which is often more useful than any one number.

Your results are saved in your Priority One dashboard, so each new draw lines up next to the last one.

Want testosterone, DHEA and cortisol too? The Women's Hormone Health panel adds free and total testosterone, DHEA-S, prolactin and morning cortisol alongside estradiol, progesterone, FSH & LH and thyroid.

Commonly added

The $10 lab service fee is charged once per order, so add-ons are just the test price — same draw, same visit.

  • Thyroid Peroxidase & Thyroglobulin Antibodies

    $13

Both thyroid autoantibodies — against thyroid peroxidase and against thyroglobulin — measured together in a single draw.

  • Free T3

    $12

The unbound, active portion of triiodothyronine — the thyroid hormone that actually does the work in tissue. Most of it is made by converting T4 outside the thyroid.

  • DHEA-S

    $14

An adrenal hormone that serves as raw material the body converts into testosterone and estrogen. It's the most abundant circulating steroid hormone and declines steadily with age.

  • Apolipoprotein B (ApoB)

    $15

Cholesterol doesn't travel through your blood on its own — it rides inside particles, and every one of those particles carries exactly one ApoB protein on its surface. Measuring ApoB counts the particles.

Common questions

Can a blood test confirm perimenopause?
Not on its own. Perimenopause is usually identified from symptoms and cycle changes over time. Hormone levels add context, and the thyroid test helps rule out a common look-alike. Talk through your results with your healthcare provider.
What day of my cycle should I test?
Cycle day 2–5 if you still have periods. If your cycles are irregular or have stopped, any day works.
Do I need a doctor's visit or referral?
No. An independent licensed physician authorizes the lab order as part of checkout. There's no appointment and no referral.
Where do I get my blood drawn?
At any Quest Diagnostics location. Your lab order arrives by email a few minutes after checkout — walk in, or book a time if you prefer. Not available in New York, New Jersey or Rhode Island.
What does it cost in total?
The test price plus a flat $10 lab service fee per order, which covers your blood draw and the physician-authorized order. The fee is charged once, no matter how many tests you add.
Can I use my HSA or FSA card?
Yes. HSA/FSA cards are accepted.
Do you take insurance?
No. Prices are cash prices, shown upfront before you check out.
When do I get my results?
Usually in about a week. We email you when they're ready (the email never includes your results), and they show up in your Priority One dashboard.
Will a doctor go over my results with me?
No. Your results go straight to your dashboard. Take them to your own healthcare provider to talk through what they mean for you.
What if I change my mind?
You can get a full refund any time before your blood is drawn.
